What is HIFU?

HIFU stands for High-Intensity Focused Ultrasound. It is a non-invasive skin tightening and lifting treatment that uses precisely focused ultrasound energy to heat tissue deep beneath the surface of the skin. The energy bypasses the outer layers entirely and reaches the SMAS layer, the same muscular layer a surgeon targets during a traditional facelift.

When the SMAS and dermal layers are heated to a specific temperature, the body responds by producing new collagen and elastin. Over the following weeks and months, that new collagen contracts and remodels, lifting and tightening the skin from the inside out.

What does HIFU treat?

HIFU is one of the most versatile non-surgical treatments we offer at our Fulham and Ealing clinics. It is most commonly used for:

  • Lifting the jawline and reducing jowls
  • Tightening loose skin under the chin

  • Tightening sagging cheeks

How does the treatment feel?

HIFU is delivered through a handheld applicator that the clinician moves across the treatment area in a precise grid. As each pulse is delivered, you'll feel small bursts of warmth and a tingling sensation deep within the skin. Most patients describe it as tolerable rather than painful, and we routinely apply numbing cream to make the experience more comfortable.

A full face and neck treatment takes around 60 to 90 minutes. There is no downtime. You can drive yourself home, return to work, or pick up the children — and apply makeup the same day.

When will I see results?

This is one of the most important things to understand about HIFU. Unlike a filler, you will not walk out of the clinic looking dramatically different. HIFU works by stimulating your body's own collagen production, and collagen takes time to build.

You'll typically notice initial tightening within the first two weeks, with the most significant lifting and contouring visible at 8 to 12 weeks. Results continue to improve for up to six months, and a single treatment can last between 12 and 18 months depending on your age, skin condition and lifestyle.

Is HIFU safe?

HIFU has been used clinically for over a decade and has an excellent safety profile when performed by a qualified practitioner. Because the energy is delivered below the skin surface, there is no damage to the outer layers, no scarring, and no significant risk of pigmentation changes, making it suitable for all skin types and tones.

Mild redness, slight swelling, or a feeling of tenderness in the treated area is normal for the first 24 to 48 hours. Rarely, some patients experience small bruises or temporary numbness, both of which resolve on their own.

Who is not suitable for HIFU?

HIFU is not appropriate for everyone. We typically advise against treatment if you:

  • Are pregnant or breastfeeding
  • Have severe skin laxity better suited to surgical intervention
  • Have active infections or inflamed skin in the treatment area
  • Have metallic implants in the treatment zone
  • Have certain medical conditions affecting healing

HIFU vs other tightening treatments

Patients often ask how HIFU compares to radiofrequency, microneedling, or thread lifts. The honest answer is that they target different depths and different signs of ageing.

Radiofrequency works in the more superficial dermis and is excellent for fine lines and surface tightening. Microneedling rebuilds collagen in the upper skin layers and is ideal for texture, scarring, and pigmentation. Thread lifts give an immediate mechanical lift but do not stimulate collagen at the SMAS layer. HIFU is unique because it reaches the deep structural layer of the face. For many patients, a combined plan is the most effective long-term strategy, which is something we'll always discuss honestly at your consultation.

How soon can I have HIFU after Botox or filler?

We typically recommend waiting two weeks after Botox and four weeks after dermal filler before HIFU, to ensure the ultrasound energy does not affect those treatments. Your clinician will plan the right sequence at your consultation.
